Whenever a patient comes into the office, it is important that you, as a provider, understand the patient’s history and have access to any information that may affect their eligibility for a procedure. Having access to a patient’s medical history before their appointment allows your practice to have a better understanding of your patients’ needs and provides a smoother check-in process. Remedly makes it easy for patients and providers to record and understand a patient’s medical history and ensure up-to-date records.
Configuring Medical History
To configure Medical History, begin by navigating to the Medical History tab under Practice Account.
From here, select the middle tab labeled Medical History.
To turn on/off different questions, use the toggle switch to select either Ask or Hide for each question.
Additionally, you can require patients to complete the text field provided when they respond "Yes" to some of the questions. To activate, use the corresponding toggle switch on the right side of medical history.
By activating this toggle switch, the patient won't be able to submit their medical history until the text input field has content.
Note: The toggle button is available only for questions that:
- are toggled to Ask
- provide the additional input text field if patient answers in the affirmative.
To create custom questions, select Add Custom at the bottom of the default questions, type in your question and select Save. Be sure to format your custom question in the same style as the preceding questions.
If the patient answers Yes to any custom question, an empty input field will appear below the question to capture additional information. Craft your custom question to guide the patient what additional information they should provide, such as Do you smoke? If so, how often?
Viewing and Recording Medical History for Staff
To see or contribute to a patient’s Medical History records, navigate to their Individual Patient History page.
To upload a medical history document, select Add New Scan and either take a picture or upload the document from your desktop using PDF, PNG or JPEG format.
To have a patient complete the medical history form onsite, select the Med History button ideally on a tablet. Be sure to have them fill out all three steps fully and sign at the bottom.

Viewing and Recording Medical History for Patients
If you have your patient portal activated, patients should be encouraged to visit the patient portal before their appointment in order to update their medical history and complete any required consent forms. The patient will also be able to view all previously submitted medical history records under Appointment Prep section.
