Leads Added through the Contact Us Form
Each provider has a unique URL for generating leads to their practice. Embedding this link into the practice website provides a tightly integrated experience for potential patients.
To determine your provider's unique URL, note the patient invitation code on the provider's dashboard which is their unique identifier.
The provider's unique URL is configured by using the core URL https://app.remedly.com/contact-us/ followed by the unique patient invitation code. In our case it is 22722.
For this example, the provider's unique URL is: https://app.remedly.com/contact-us/22722
Use this link to display the Contact Us form within the practice website - it embeds easily within an iFrame for maximum flexibility.
When a prospective patient completes and submits their information using the Contact Us form, they are added to the Remedly platform as a new lead which displays as a dashboard notification for all staff assigned to receive New Lead notifications.
Note: In order to get dashboard notifications you need to turn this feature on in Practice Account -> User Accounts -> choose the user who needs to see the notifications -> click Edit -> expand Dashboard Notification section in Global permissions-> turn on the toggle.
NOTE: The user will also receive a default email alert:
The staff member can review the new lead information, contact them, assign the lead to another staff member for follow-up, and add notes to track their communications.
This can be done from 3 places:
1. By clicking Review in the dashboard
2. By going to Marketing -> Current Leads
3. By clicking Current Leads in the scheduler
IMPORTANT:
If there is already an existing patient with the same email address and/or phone number, then:
- users set up in User Accounts to be notified of new leads will receive a Contact Request dashboard notification so they can follow up with the patient.
- a new lead account will NOT be created in the Current Leads section of the Marketing module

Managing New Leads
How to Review New Leads
To review all of the current leads, use any of the following ways to access them:
- Main Menu > Marketing > Current Leads
- being on the Calendar click on the Current Leads icon in the menu right above the Calendar
- being on the Calendar click on Marketing > Current Leads
Current Leads provides an overview of all current and archived leads, displays the status of your lead communications, and allows leads to be assigned to other staff, among other capabilities.
If you're not checking Current Leads frequently and want to be reminded to follow up with a lead, create a reminder to send yourself a dashboard notification.
How to Add New Leads Manually
Leads can be manually added if the prospect walked or called into the practice using the New Lead button.
To accomplish this, follow the next steps:
1. You can access Current Leads in 3 different ways:
- Main Menu > Marketing > Current Leads
- being on the Calendar click on the Current Leads icon in the menu right above the Calendar
- being on the Calendar click on Marketing > Current Leads
2. In the pop-up window hit New Lead.
3. In the new pop-up window choose the Referral source, type in First Name, Last Name, and Cell and/or Email, and hit Create.
4. If the email address and/or phone number is used by an existing patient you'll be notified by this error message. To dismiss the message, click on the OK button.
Users set up in User Accounts to be notified of new leads will simultaneously receive a Contact Request dashboard notification.
Converting Leads into Patients
A lead converts into a patient when they have their first appointment scheduled. In order to track that conversion, it is important when scheduling the appointment to select the lead via the patient input field.
If a new patient account is created instead, then the lead will not be tracked as converted.
Lead Conversion Report
The Lead Conversion report shows your top referral sources for converted leads. This report is located within the Reports module under Patient Activity then Patient Conversion Reports.
In the Patient Conversion Reports Section choose the date range you are interested in and select Leads Conversion.
After that click Create report.
Use the Print PDF button to see more details including patients names and revenue breakdown.

If you want to check concrete referral source, you can select the once you need using the pull down menu. Only selected referral sources will be included in the results.
