Consent forms are any documents which patients need to review and sign ideally before their appointment. Forms are typically consent forms but they can also be legal documents or any documents which patients need to review and sign.
Forms are added to Remedly during practice onboarding. If you need to add a new form, please contact support@remedly.com.
If consent forms are mapped to a chosen service, they will be automatically added to any appointment for that service. The forms mapping to services is done during the onboarding process by the Remedly team.

Signing Consent Forms
From the patient's side
When you schedule an appointment with services that are mapped to consent forms, an email with the links to the associated consent forms will automatically be sent to the patient.
The patient will be able to complete the form by clicking on the links provided in the email.
From the provider's side
If a patient has not signed their forms prior to their appointment, they can complete this task while at the practice. We encourage the front desk to have a dedicated tablet for this purpose.
From the appointment, simply click the 3 dots near the desired form and then click Sign.
The consent form will open in a new window for the patient to review, sign, then Save.
The consent form will be marked as signed in the appointment.
Accessing Consent Forms
From the appointment
Consent forms as well as their status are visible by opening the appointment window.
The status icon displays if the consent form is signed or not.
A notification will display if there are unsigned consent form.
Consent forms can be resent to the patient via email by clicking on the 3 dots near the desired consent form and selecting Resend via email.
Consent forms can be removed from the appointment by clicking on the 3 dots and then Remove from appointment.
From the patient chart
To check the status of the overall forms sent to the patient to see whether they were signed or not or refer to previously submitted forms you can find them in Patient Chart under Forms.
To view the forms of the patient:
- Find the required patient and go to the Patient Chart>Forms
- Find the necessary form in the list and click any form to open it.
