When a consent form is added to Remedly and tied to the appointment, the form will be sent automatically to their patient portal when an appointment is created for the corresponding procedure with the appropriate appointment type (initial consult, pre-treatment, treatment, or follow-up).
Please note that patients do not receive an automated email reminding them to complete their forms.
In order to remind patients to complete their forms in their patient portal, consider adding a reminder to your Automated Communication emails such as "Be sure to complete your medical history and consent forms in your patient portal prior to your appointment."
