To access all details regarding a patient's engagement with your practice, navigate to their Patient History page. In this article you'll get acquainted with the many elements of the Patient History page.
In this article you will learn how to:
- Search for patient accounts
- Navigate the top right tile including patient demographics, allergies/medications, and engagement history
- Navigate to the bottom right tile for notes and alerts
- Navigate to the left side tile for patient documentation and demographics
Patient Search
To access Patient History, type the patient's first or last name, email, date of birth, or phone number in the patient history search field located:
- On the dashboard,
- In the calendar,
- or on a patient history page.
Once on the patient history page, there are three tiles of data.
Top Right Tile
Every patient has an ID which is displayed after the patient's name. Below the patient ID, the referral source indicates the most recent input of how this patient found your practice.
The most relevant patient data is available at a glance: date of birth, age, contact phone, email, and address.

For patients enrolled in a Membership Program, the Membership Label is visible on their patient chart under the patient's name.
To generate a printed version of patient demographics, click Face Sheet. The information is presented in PDF format in a separate tab for downloading and printing.
Select Show Summary of All Notes to view committed notes from previous appointments.
It is important to document all patient Allergies to ensure their consideration when providing care. Allergies data is populated from patient-completed medical history and, if eRx is activated, from RCopia data.
Note: If your practice has enabled DrFirst, the allergies from RCopia will also be populated in the medical history. So If you see allergies added in the medical history, but didn't add them in Remedly, they are most likely to be from RCopia.
To ensure a patient's current medications are being considered when providing care, the Patient History page provides the ability to add/edit a patient's Medication, with the information highlighted for visibility.
Note: with integrated eRx, it is best to document medication within Rcopia so contraindications are highlighted. There is no data integration between Remedly and Rcopia for medications.
Engagement history provides a summary of every activity the patient has had over the course of time with your practice.
Choose the date range to see the list of events during the selected time period. The events include all appointments and financial transactions.
In order to see at a glance which providers a patient has previously seen, Engagement History includes the provider name for each appointment.
Click View to open the:
- appointment window or
- PDF of the financial transaction
Bottom Right Tile
The tile encompasses all notes and alerts. Here you can add any information about the patient you find important, and the contents are only visible to you and your colleagues. Notes are visible only on the patient history page but alerts will display whenever an appointment is created or opened for the patient.
To add or edit the notes, click Add/Edit.
In the new window, add the note contents in the input field, mark it as an alert if needed, then click Save.
To edit a note, find the note and click Edit. Change the note contents and click Save.
Left Side Tile
This section is where the patient documentation and demographics can be found and managed.
To edit patient demographics, including contact information and insurance details, click the pencil icon.
To print an account statement, click the $ icon and input the date range needed to render a PDF file summarizing all of the debits and credits on the account for that time period.
Procedure and Chart Notes provides access all procedure and chart notes created for the patient. Use the filter to see only active, archived or all notes and click PDF to open the note. You can also upload a scanned note or create a new note.
Note: if a provider is documenting their note from the appointment window, staff can create notes for the same appointment from patient history.
Providers can either approve notes to commit them or amend committed notes to add more information.

Estimates section provides a summary of all patient quotes and their status. Use the filter to see estimates based on their status.
You can create a new estimate, open a PDF of an estimate, or edit an existing estimate. You can also archive estimates no longer needed. Note that estimates cannot be deleted, only archived.

Invoices section provides a summary of all invoices for services rendered and their status: paid, balance due and voided invoices. If the patient has an outstanding balance, it will be highlighted in red. To understand why the patient has an outstanding balance, run an account statement to see all of the debits and credits on the account.
Click PDF to view the invoice or Edit to adjust the invoice. Clicking the X button will void the invoice.
Note: Refrain from editing or voiding invoices after a payment has been generated against the invoice as this may cause account balance issues.

Payments & Refunds provides a summary of all payments applied to the account as well as payment refunds.
To put credit on a patient's account to be applied towards any future invoices, select Payment and complete the payment collection. The total amount of credit available on the account will be highlighted in red.

To refund any previous payment, use the Refund button.

Clicking the X button will void any payment or refund.
Note: the void option is only available if the payment was not rendered via integrated merchant services, and the void only occurs within the Remedly platform for reconciliation.
Hovering over any payment or refund will produce more information regarding which invoice the payment or refund correlate to and other transaction details.
Messages shows the history of the correspondence with this patient via the patient portal. Messages can be filtered to display the inbox, sent or archived messages. A new message can also be created.
Prescriptions summarizes all patient prescriptions issued via DrFirst or scanned and uploaded. With integrated eRx, you can issue prescriptions using the Issue Rx button or add a scan of a prescription.

Lab Tests allows uploading of scanned lab test results as a PDF for documentation within the patient chart.
Note: any uploaded lab tests are automatically available to the patient via the patient portal.
Forms include all documents requiring patient reviewing, input, and signatures, such as consent forms, post-treatment care forms, and any legal documents. Once signed, all forms are available in this section as PDFs. New forms to be signed can be initiated using the Complete Forms button. Forms can also be uploaded in PDF format.

Online New Procedure Consultations and Online Follow-ups initiated via the patient portal can be viewed in this section. The status of each patient engagement can be viewed as well as a PDF summary.
Before & After Photos allows providers to share with the patient the impact of their treatment(s). Click Add/View Photos to see all the uploaded photos as well as add new photos.
Note: photos taken in the note while using the sketching tool are not added automatically to this section but can be downloaded from the note and uploaded to this section if desired.

Medical History retains all submissions of patient medical history as unique PDFs. Patients can complete medical history from the patient portal as well as from the Medical History button in this section. Medical History scanned as a PDF can also be uploaded.
Note: only scanned medical history documents can be deleted by clicking the X button . All other medical history documents cannot be deleted or archived to ensure a complete patient record.
Memberships is a comprehensive overview of all patient's membership enrollments and details as well as the ability to adjust enrollment statuses.

Packages summarizes the status of all Prepaid Packages purchased by the patient. The packages can be filtered to show only active, completed, cancelled, or all packages. The PDF button will open the invoice of the Prepaid Package purchase. The balance of the remaining treatments or inventory available in each package is displayed on the right. For example, 4/5 means that 4 out of the original 5 procedures purchased remain available to the patient to receive.
When the package is fully depleted, the prepaid package is considered completed.

Credit Cards retains a summary of all credit cards on file and allows uploading of new credit cards if integrated merchant services is enabled. There is no limit to how many credit cards can be retained on file. These credit cards can be used for rendering payments as needed.
